Our growing national conference, taking place at the Westin Boston Seaport District, is the ideal opportunity for DRVS users to meet, share best practices and ideas, further knowledge and skills, network with the Azara team, and each other.

The Azara User Conference is designed to empower attendees to maximize their use of Azara tools to drive improvements in care delivery and positively impact patient lives. Sessions cover topics appropriate for all levels of DRVS users—new to advanced—and aim to broaden and deepen the use of solutions and modules.

Project ECHO Spring Session 8

Case Management for Substance Use

It can be quite challenging for those with substance use disorders. Individuals with substance use disorders (SUDs) commonly have concurrent mental and physical health problems and face multiple barriers to treatment. Many individuals struggle with homelessness, job loss, lack family support, lack financial stability, and may have criminal justice involvement, in addition to the struggles of addiction.

This session will focus on the effective use of case management services for individuals with substance use disorders. Topics will include social determinates of health (SDOH) and why SDOH are important to understand and address the barriers to treatment/recovery. Additionally, the session will discuss the differences between case management and care management, the types of case management models, why case management is important to the addiction treatment and recovery, with best practices.

The passage of the 2018 Farm Bill allowing the production of hemp products created a controversial loophole in the law which seemingly permitted businesses to sell consumable, intoxicating hemp products. As a result, the sale and recreational use of these hemp derived intoxicants increased dramatically in 2020 and continues to remain steady. Many Wyoming communities have seen the recent opening of stores and existing stores selling these products locally. While Delta-8 THC is the most common hemp derived intoxicant product, may other products exist producing intoxicating effects in consumers similar to legalized cannabis and other illicit drugs. This presentation will cover the multitude of hemp derived intoxicants currently available on the market, the psychoactive effects of these products, and the public health and safety concerns regarding these products due to a lack of federal and state regulation.

A Look at Behavioral Health Integration through the Collaborative Care Management Model

As stated by CMS in the March 2021 MedLearn, “Integrating behavioral health care with primary care (behavioral health integration or BHI) is now widely considered an effective strategy for improving outcomes for millions of Americans with mental or behavioral health conditions.” Collaborative Care Management (CoCM) is a care delivery model that can enhance care for patients enrolled in Behavioral Health Integration. With the limited number of psychiatric medical providers, utilizing CoCM to obtain recommendations for these patients is an excellent solution. Join us to learn how a Wyoming Rural Health Clinic is utilizing their BHI care management program to include CoCM by partnering with a Psychiatric Nurse Practitioner to conduct weekly case reviews. Hear from the NP and the RN care coordinator as they highlight the process and share how this model has positively impacted the patients. Additionally, there will be an overview of how such a program is sustainable under the reimbursement models of Medicare and Wyoming Medicaid.
